CODE OF CONDUCT REVIEW FORM
Last year every parent, guardian and student in grades 6-12 received a copy of the District’s Code of Conduct. The Code was recently approved by the Board, as is done annually, with no revisions from last year. In the interest of cost and paper savings, we are not distributing copies of the Code again this year to student and families.
The Code is available on the District’s website (
www.nyackschools.org under Quick Links) and in each of the schools. Families of kindergarten students will receive a copy of the Code and this form at Parent Teacher conferences. New registrants at the elementary level will receive it in their backpacks and it will be mailed home for those in the middle and high schools.
Since the Code is an important district policy with which everyone should be familiar, administrators are still visiting all classrooms to review it with students. We are also requesting that you sign the form below stating that you have reviewed the document and then please return the form to your child’s schools office as soon as possible.
If you do not return this form signed, it does not with the Code. release you or your student from compliance
